BENEFITS SUMMARY
With a primary focus on employee satisfaction, PDR, Inc. works diligently to provide benefits aimed at enhancing quality of life. Benefit packages can be tailored to accommodate personal choices and circumstances. To follow is a summary of our offerings:
HEALTH INSURANCE:
PDR has partnered with Humana (

) to offer our employees a cost effective and flexible Point of Service health insurance plan. Members can stay within the network and pay a lower share of the cost – or they can seek services outside the network and pay more. The employee’s contribution to the Plan is made through pre-tax payroll deductions that reduce taxable income and tax liability.
DENTAL INSURANCE:
PDR offers an employee paid group dental insurance program through Dental Care Plus (

) Dental Care Plus is developed and owned by dentists in Southwestern Ohio, Southeastern Indiana and Northern Kentucky. It has successfully combined the best aspects of conventional dental insurance with the efficiencies of a health maintenance organization. The employee’s contribution to the Plan is made through pre-tax payroll deductions that reduce taxable income and tax liability.
SECTION 125 PLAN:
This is a free benefit to employees. By enrolling, employee premiums for medical and dental coverage are deducted from the employee’s paycheck before taxes, reducing taxable income and the amount of tax you pay. In addition, the employee may elect to have additional monies withheld pretax to be reimbursed upon submission of a claim form and receipts for other qualified medical expenses as well as dependent care costs.
LIFE INSURANCE:
PDR has partnered with Assurant (

) to provide a Group Term Life and Accidental Death and Dismemberment program to all employees at no cost. The plan pays 1 x annual salary to a maximum of $50,000.00, double indemnity in the event of accidental death or dismemberment. Employees also have the option to purchase additional coverage at discounted rates for themselves, their spouse and/or their dependents.
DISABILITY INSURANCE:
PDR has partnered with UNUM (

) to provide Long Term Disability insurance to employees at no cost. Employees also have the option to purchase Short Term Disability for minimal cost.
PAID TIME OFF:
PDR believes that employees should have opportunities to enjoy time
away from work to help balance their lives. Following are ways that PDR gives their employees
the opportunity to do so:

Holidays:
PDR observes 8 holidays which include New Year's Day, Memorial Day,
Independence Day, Labor Day, Thanksgiving Day, Day after Thanksgiving, Christmas Eve, and
Christmas Day.
Personal Days:
Employees may earn up to four personal days each year. This time is
to be taken for illness, family emergency, and any other necessary absence.
Vacation:
Vacation is earned on an accrual basis and generally follows a
seniority scale 0-5 yrs of service = 2 weeks, 5-10 yrs of service = 3 weeks, 10+ yrs of
service = 4 weeks
Bereavement:
Employees receive three days of bereavement pay in the event of a
death of a member of the immediate family.

OVERTIME:
Employees are paid at their hourly rate for every hour worked and approved by the
client.
EMPLOYEE REFERRAL BONUS:
As a highly qualified technical professional, we feel there is no better judge of
a person’s credentials than to have one of our employee’s attest to their competence. To reward you
for this effort, we will compensate you with a referral bonus of $1,000.
EDUCATION AND TRAINING:
PDR offers a variety of training and development options to foster an environment of
personal development. Tuition reimbursement of up to $1,000.00 is allotted each year on an accrual
basis for industry-related courses required for industry-related, university degree programs. In
addition, PDR may offer financial assistance for those interested in technical training.
401(k) PLAN:
PDR's 401(k) Plan gives employees the opportunity to save for retirement on a
before-tax basis. In addition to the benefit of accumulating tax-free income in your 401(k)
account, PDR may make a contribution for each dollar you deposit into the Plan up to the legal
limit. All earnings accumulate tax-free until withdrawn. Eligibility to receive Company matching
contributions and interest is based on a six year vesting schedule. American United Life Insurance
Company (

) is the chosen plan provider.
AUTO AND HOME INSURANCE DISCOUNTS:
PDR has teamed up with Liberty Mutual to offer our employees Group Savings Plus®.
This unique program provides high-quality auto, home, and renters insurance at
low group rates.
EMPLOYEE BENEFIT BANKING:
PDR has partnered with CINFED Credit Union to offer a unique package of financial
services created for our employees, providing a full range of money saving banking options.
COLLEGE SCHOLARSHIP:
PDR offers a college scholarship for the children of employees. The fund is
available to children of employees who have been employed by PDR for a minimum of two years and are
a freshman, sophomore, junior or senior in college.
COLLEGEADVANTAGE 529 SAVINGS PLAN:
PDR works with the Ohio Tuition Trust Authority to allow employees the opportunity
to invest in a college savings program that enables individuals to save and invest on a tax-favored
basis in order to fund future higher education expenses of a child or other beneficiary.
